Pharmacologically, melanotan-2 behaves as a non-selective agonist across the melanocortin receptor family. Binding at MC1R on dermal melanocytes promotes eumelanin synthesis, which underlies the tanning response described in early human work. Activity at the centrally expressed MC4R receptor is associated with reported effects on appetite and erectile function. Because the peptide does not discriminate strongly among receptor subtypes, attributing any single observed effect to one receptor pathway is generally not possible without selective antagonists or receptor knockout models.
Research interest has centred on photoprotection and pigmentation, with a smaller body of work on appetite and sexual function. Published human data remain limited to small, frequently uncontrolled studies, and the compound has never received marketing approval from a national medicines regulator. Most laboratory work treats it as a pharmacological tool for probing melanocortin signalling in cell culture or animal models. Whether pigmentation changes observed in people translate into measurable protection against ultraviolet-induced DNA damage remains an open question.
Melanotan-2 is a synthetic cyclic heptapeptide designed as an analogue of alpha-melanocyte-stimulating hormone, the naturally occurring peptide involved in pigmentation signalling. Its sequence is conventionally written as Ac-Nle-cyclo[Asp-His-D-Phe-Arg-Trp-Lys]-NH2, with a lactam bridge joining the aspartate side chain to the lysine side chain. The empirical formula is C50H69N15O9 and the monoisotopic mass lies near 1023.5 daltons. N-terminal acetylation and the D-configured phenylalanine both increase resistance to enzymatic breakdown compared with the parent hormone.
Identification in laboratories relies on reversed-phase liquid chromatography coupled with tandem mass spectrometry, with product-ion spectra compared against a certified reference standard. High-resolution mass spectrometry supplies accurate mass confirmation, and peptide mapping after enzymatic digestion separates melanotan II from closely related analogues. Quantitation of seized material is complicated by unknown counter-ions and residual trifluoroacetate left from purification. Immunoassays raised against alpha-melanocyte-stimulating hormone can cross-react, so chromatographic confirmation is normally required. Urinary detection windows are short, and reported limits of detection differ substantially between laboratories.
Melanotan II holds no marketing authorisation from the Food and Drug Administration, the European Medicines Agency, the UK Medicines and Healthcare products Regulatory Agency or Australia's Therapeutic Goods Administration. Products sold under that name are treated as unapproved new drugs, and their sale or import is prohibited in several jurisdictions. Other countries classify the peptide as a prescription-only medicine or place it among controlled substances, so the legal position changes with the destination market. No pharmacopoeial monograph supplies an official specification, because the material is not a licensed pharmaceutical. Consequently, products offered online are not manufactured to a shared public standard.
The peer-reviewed record is dominated by small early-phase studies, case reports and pharmacovigilance summaries rather than large randomised trials. Papers typically examine tanning response, receptor selectivity or patterns of reported adverse events. Many note that participants obtained the peptide outside a clinical setting, which limits verification of composition and administered amount. Reported events vary widely, and causality is frequently unclear because the identity and purity of self-sourced material are unknown. Open questions include whether repeated melanocortin receptor stimulation produces cumulative effects, and how often label claims match actual content.

The peptide acts as a non-selective agonist at melanocortin receptors, showing affinity for MC1R, MC3R, MC4R and MC5R. Activation of MC1R on melanocytes drives the conversion of tyrosine into melanin and shifts production toward the darker eumelanin form. MC4R signalling in the central nervous system is linked to appetite and energy balance, which helps explain why reduced food intake appeared in early human studies. Effects on MC4R and on vascular tone also account for the erectile responses recorded as unexpected findings in those same trials.
Melanotan-2 is frequently confused with afamelanotide, a linear analogue authorised in the European Union for erythropoietic protoporphyria. The two compounds differ in chain length, ring structure and receptor selectivity, so findings for one cannot be transferred directly to the other. Published controlled human data on melanotan-2 remain sparse, and much of what circulates online derives from small studies or unpublished reports. Questions about effect size, dose-response behaviour and long-term safety therefore remain unresolved.
Because the substance circulates mainly through informal markets, verification is a recurring theme in technical discussion. Independent analyses have found that labeled content and actual content can diverge, and that purity varies between samples. Analytical laboratories use reversed-phase chromatography to separate components and mass spectrometry to confirm identity. Isotope-labeled internal standards improve quantification in complex matrices. Such methods describe what a sample contains but say nothing about its sterility, lawful status, or suitability for any use. Open questions remain about how consistently testing is applied across the supply chain.
Regulatory treatment of this peptide varies by country. It holds no marketing authorization as a medicine in the United States, the European Union, or most other jurisdictions. Some countries classify products containing it as prescription-only or unlicensed medicines, which restricts lawful supply. Authorities have issued public notices warning that unregulated products may contain undeclared or incorrect ingredients. The molecule also appears on prohibited lists for competitive sport. These measures address supply oversight rather than any approved therapeutic role.
The compound was developed in the late 1980s and 1990s by academic researchers investigating photoprotection. The rationale held that stimulating melanin production might reduce ultraviolet damage to skin and lower skin cancer risk. Early work examined receptor binding, pigment response, and short-term tolerability in small studies. That program did not produce an approved drug, and formal development stalled after early-phase trials. Whether induced pigmentation confers meaningful photoprotection remains an open question.
Outside regulated medicine, melanotan II circulates through online vendors as a research chemical, often marketed for tanning. Products sold this way vary widely in purity, concentration, and labeling accuracy, and independent testing has documented discrepancies. Published reports describe both pigment effects and adverse reactions, including nausea, flushing, and darkening of existing moles. Long-term safety data are sparse, and no large controlled trial has established a risk profile. Questions about cumulative effects on melanocytes remain unresolved in the literature.
Ketobemidone, sold under the brand name Ketogan (a mixture of ketobemidone and Spasmolytic A29) among others, is a powerful synthetic opioid painkiller. Its effectiveness against pain is in the same range as morphine, and it also has some NMDA-antagonist properties imparted, in part, by its metabolite norketobemidone. This may make it useful for some types of pain that do not respond well to other opioids. It is marketed in Denmark, Iceland, Norway. Until 2024 it was available in, but is now withdrawn in Sweden. It is used for severe pain.
Acta Biochimica et Biophysica Sinica was a Chinese-language journal entitled "Shengwu Huaxue Yu Shengwu Wuli Xuebao" (see box below), which began publishing sometime in 1958 or 1959, or August 1961, until December 2003 (depending on the source). Tables of contents were in English. Summaries (abstracts) were in English and Russian, and text of articles in Chinese. Later issues were promulgated in the Chinese and English languages. The translated title was "Journal of Biochemistry and Biophysics". The frequency was quarterly from 1959 to 1963, bimonthly from 1964 to 2002, and finally, monthly by 2003. Publication was suspended from 1966 to 1975. It was indexed in Chemical Abstracts Service. It may have published 35 volumes in total, over the years. In 2004 the journal switched to a full English-language journal under its current title. First observations of short-lived pear-shaped atomic nuclei Research conducting using the Miniball experimental setup found evidence of pear-shaped heavy nuclei, in particular radon-220 and radium-224. These results were named in the Institute of Physics (IoP) "top 10 breakthroughs in physics" in 2013, and was featured as the cover of Nature 2013. In 2020, due to the HIE-ISOLDE upgrade, radium-222 was also found to have a "stable pear shape". Laser spectroscopy has been performed on a short-lived radioactive molecule, containing radium, which further studies into could reveal physics beyond the Standard Model due to time-reversal symmetry breaking. Measurement of 229mTh transition energy In 2023, ISOLDE made the first 1%-level measurement of the ultralow-energy thorium-229m nuclear isomer, detecting photons at an energy of 8.338±0.024 eV. This was a key step in the construction of a future nuclear clock. Below is a list of improvements needed for the ISOLDE facility, considering both medium and long-term goals. Some of these improvements have been proposed by the EPIC project.
== History == Midazolam is among about 35 benzodiazepines currently used medically, and was synthesized in 1975 by Walser and Fryer at Hoffmann-LaRoche, Inc in the United States. Owing to its water solubility, it was found to be less likely to cause thrombophlebitis than similar drugs. The anticonvulsant properties of midazolam were studied in the late 1970s, but not until the 1990s did it emerge as an effective treatment for convulsive status epilepticus. As of 2010, it is the most commonly used benzodiazepine in anesthetic medicine. In acute medicine, midazolam has become more popular than other benzodiazepines, such as lorazepam and diazepam, because it is shorter lasting, is more potent, and causes less pain at the injection site. Midazolam is also becoming increasingly popular in veterinary medicine due to its water solubility. In 2018 it was revealed the CIA considered using midazolam as a "truth serum" on terrorist suspects in project "Medication".
== Clinical trials == The efficacy and safety of ipragliflozin were both observed in a Phase III study in monotherapy and clinical studies used in combination with other hypoglycemic agents (6 types) in Japan. One placebo-controlled, double-blind study was carried out at 18 different sites in Korea and 12 in Taiwan. Patients were above 20 and had type 2 diabetes for atlas 12 weeks. They were given an 8-week period to clear their systems of all other drugs (besides metformin). Patients received either 50 mg ipragliflozin or a placebo. The drugs were identical in all physical forms. The patients were prohibited from using any other anti diabetic drugs, other than metformin. The study ran for 24 weeks along with a 4-week follow-up period. The standard deviation in hemoglobin A1c were −0.94% and −0.47% in the ipragliflozin and placebo groups, respectively (between-group difference −0.46%, p <0.001). The changes in fasting plasma glucose and bodyweight were also significantly greater in the ipragliflozin group, with between-group differences of −14.1 mg/dL and −1.24 kg, respectively (both p <0.001). The most common adverse events that appeared were upper respiratory infections and urinary tract infections. From this it was concluded that ipragliflozin is both efficacious as well as safe. However, ipragliflozin is currently in Observational Case Control clinical trial to see the long-term (over three years) safety of using ipragliflozin. The estimated completion of this trial is October 2018.

=== Liver toxicity and urologic disease === Urologic diseases occur primarily in people who use large amounts of ketamine routinely, with 20–30% of frequent users having bladder complaints. It includes a range of disorders from cystitis to hydronephrosis to kidney failure. The typical symptoms of ketamine-induced cystitis are frequent urination, dysuria, and urinary urgency sometimes accompanied by pain during urination and blood in urine. The damage to the bladder wall has similarities to both interstitial and eosinophilic cystitis. The wall is thickened and the functional bladder capacity is as low as 10–150 mL. Studies indicate that ketamine-induced cystitis is caused by ketamine and its metabolites directly interacting with urothelium, resulting in damage of the epithelial cells of the bladder lining and increased permeability of the urothelial barrier which results in clinical symptoms. Management of ketamine-induced cystitis involves ketamine cessation as the first step. This is followed by NSAIDs and anticholinergics and, if the response is insufficient, by tramadol. The second-line treatments are epithelium-protective agents such as oral pentosan polysulfate or intravesical instillation of hyaluronic acid. Intravesical botulinum toxin is also useful. Some research also indicates that epigallocatechin-3-gallate (EGCG) may mitigate bladder dysfunction in ketamine-induced cystitis by normalizing the collagen-to-muscle ratio and restoring storage capacity. Hepatotoxicity (toxicity to the liver) of ketamine involves higher doses and repeated administration.
Changes in nearby tissue is not the only possible impact of ligand therapy, there may be immunologic responses from the target tissue that cause changes remotely. This has been dubbed the "abscopal effect". While this mechanism is not well understood, it explains the impact of other tissue, both benign and malignant, after targeted radiotherapy.
An error in choosing the individuals or groups to take part in a study. Ideally, the subjects in a study should be very similar to one another and to the larger population from which they are drawn (for example, all individuals with the same disease or condition). If there are important differences, the results of the study may not be valid. (NCI) Serious Adverse Event

"My mind was, that a dogma was an idea for which there was no reasonable evidence. You see?!" And Crick gave a roar of delight. "I just didn't know what dogma meant. And I could just as well have called it the 'Central Hypothesis,' or — you know. Which is what I meant to say. Dogma was just a catch phrase."
=== Pharmacokinetics === Amitriptyline is readily absorbed from the gastrointestinal tract (90–95%). Absorption is gradual with the peak concentration in blood plasma reached after about 4 hours. Extensive metabolism on the first pass through the liver leads to average bioavailability of about 50% (45%-53%). Amitriptyline is metabolized mostly by CYP2C19 into nortriptyline and by CYP2D6 leading to a variety of hydroxylated metabolites, with the principal one among them being (E)-10-hydroxynortriptyline (see metabolism scheme), and to a lesser degree, by CYP3A4. Nortriptyline, the main active metabolite of amitriptyline, is an antidepressant on its own right. Nortriptyline reaches 10% higher level in the blood plasma than the parent drug amitriptyline and 40% greater area under the curve, and its action is an important part of the overall action of amitriptyline. Another active metabolite is (E)-10-hydroxynortriptyline, which is a norepinephrine uptake inhibitor four times weaker than nortriptyline. (E)-10-hydroxynortiptyline blood level is comparable to that of nortriptyline, but its cerebrospinal fluid level, which is a close proxy of the brain concentration of a drug, is twice higher than nortriptyline's. Based on this, (E)-10-hydroxynortriptyline was suggested to significantly contribute to the antidepressant effects of amitriptyline. Blood levels of amitriptyline and nortriptyline and pharmacokinetics of amitriptyline in general, with clearance difference of up to 10-fold, vary widely between individuals.

It is a synthetic cyclic heptapeptide analogue of alpha-melanocyte-stimulating hormone, containing seven amino acids with a lactam ring and a D-configured phenylalanine residue. It is supplied as a lyophilised powder for laboratory research.
The natural hormone is a linear tridecapeptide that is rapidly degraded in circulation. Melanotan-2 is shortened, cyclised, N-terminally acetylated, and carries a D-amino acid substitution, all of which slow enzymatic breakdown.
No national medicines regulator has approved melanotan-2 for therapeutic or cosmetic use. In several jurisdictions it is treated as an unapproved prescription medicine, and its legal classification differs from country to country.
Regulatory treatment varies by country. In the United States, the European Union and Australia it is an unapproved drug and its sale is restricted, while some other jurisdictions list it as prescription-only or controlled. The applicable rules depend on the country of import.
